you will be asked for circumcision by a doctor.
Then in next hours you have to follow 5 time prays in Masjid.
And he will further guide you about other Islamic practices.
Due to change of religion , if your are married , your marriage will remain continued. No side effects.

Kalma Tayyaba means faith in supermacy of Allah Pak and faith that Hazarat Mohammad PBUH is his LAST prophet .

for changing name in records , you can ask the concerned department their requirement for changing name. an information for public in a news paper for changing name is must.
