Change in name in birth certificate

Hello Everyone,

I need a suggestion regarding changes required in DOB Certificate dated 28 may,1993 issued from Munipality,Mandi Dabwali,Distt. Sirsa,Haryana.

The Name in Birth Certificate is not actual & require changes.

The Name in Pan,Licence,Aadhar Card is actual ie Nitin Garg,instead of Akhil garg inBirth Certificate.

I have also approached muncipality authorities,they have told me that there is not such rule for changes in Birth certificate once formed.

Also shown them this link where a method for change in birth certificate by an undertaking is suggested.
Please suggest me what to do in this situation....Require this as early as possible...as a must document for my passport...

In your tag it is mentioned as follows.

Changes in the certificate can be undertaken at same registration unit on a written request along with an affidavit and undertaking from the informant.

Sir, I have approached them with same affidavit several times.But they are saying that there is no such provision for name change in birth certificate.If you are saying that an affidavit with an undertaking is sufficient for this can you tell which higher authority other than this should I approch so that I can be able to make suitable changes in my birth certificate.

You can file a petition in the locl civil court for declaration of name in birth certificate.

Sir,

As you know the results from the petition will take much time and i have to submit these documents for my passport as early as possible.
