Birth Certificate

Please advise that my issue is I want to add name of my daughter in birth certificate but father/mother's surname is missing from already issued birth certificate whereas it exists in all other documents. Please suggest what I should do. Thanks

Dear Sir Please visit concerned office and give application for completion of parents name n addition of child name in birth certificate in the name of Registrar Birth and Death. As per rule name of child can be add till 15 year of age. Regards Rajinder Pandey Chandigarh 09814105415
