Affidavit for non claiming of income tax benefit by a home m

I had purchased a flat in the joint name of my wife who is a Home maker and does not claim any Income Tax benefit, From the home loan, Hence I want to claim the entire Tax benefit for which I need to submit an affidavit saying that my wife is non-claimant of income tax benefit. Kindly suggest the format for the same.
