Obtain obc certificate

Hi all,
I was born in Gujarat and raised in Pune. Our real surname in Gujarat is "Chudasama" and it is the same on my dad's OBC certificate. But in Pune on all our legal docs, we have our surname as "Mistry."
The question that I have is, how do I change my surname to our real surname "Chudasama"? Will I be able to obtain my OBC certificate from gujarat after changing my surname to "Chudaname?" And, If I change my surname do I have to change my dad's surname as well along with my wife and brother's?

how did chudasama become mistry? did you change your surname ? what is name in your birth certificate ?
fro change in school leaving certificate you would need court orders as you must have passed out more than 2 years back . no application is entertained by Board after lapse of 2 years for correction of surname

Try to get the OBC certificate from the competent authority on the basis of the caste. If the authorities does not accede to your request take help of court.

Hi Ajay Sethi,
I do not have a copy of my birth certificate as i was born in 1986 when we use to get only 1 certificate which school takes it.
There was no change of surname done legally. While getting an admission in school my parents asked to put surname as mistry and that is how later it became mistry on all documents even for my parents and brothers.
I have completed my schooling long back.

if you wan to change your name from dhaval mistry to dhaval chudasama then you can have change of name published in official gazette and 2 local newsspapers.
also make an affidavit regarding change of your name. You have to mention therein your specific reason for changing your name

So, does this mean official gazette and affidavit are two different things?
I am ready to do anything but i also want to be sure that all that i do will or can help me to get the caste certificate.
