Who is responsible and what is the remedy?

I am asking in concern of Official Language cadre in Govt of India, despite the clear accepted recommendations by parliamentary committee on official language and orders by the President of India, no progress have been seen towards the restructuring, promotional prospects ,effective implementationand in reforming the working conditions for so many years. Is there any accountability?
