Every authority has to designate one authority for receiving notices and settlement of grievances and should publish/display the act and the name of designated official in a conspicuous place in the notice board. First make a call to HRD / Personnel dept and seek specific guidance or contact your erstwhile colleagues for expediting the issue.

While you submit FormI, narrate that 'Notice of Determination of Gartuity', payment was never supplied despite representations in office and also that FormI and neccessary guidance to fill and submit for payment was not provided to you.
