Format of a restoration petition (civil)

Hi,

Can any one guide me how a dismised CWP (PIL), which is dismissed due to default (petitioner not present) on its first hearing, can be restored. I want to know the procedure and the format of the Restoration Petition to be filed in the Punjab & Haryana Hifh Court.
