document clarification

I have a registered marriage certificate where my name appears as per my passport records. I have a query can someone please help based on this registered marriage certificate can I change my surname and incorporate my hubby's name as my surname in my passport or do I need some additional documents to change surname For eg my name is.abc and my hubby is xy I want to change my name from.abc to abx what additional document I need. Thanks

execute an affidavit before the Notary that your name is abc before the marriage and you are being called as abx after marriage but the person abc and abx is one and the same.(and also you may write your name as abc alias abx ) and the affidavit given by you shall be attested by the Notary. this is for information.

Thank you for your response but I have 2 queries 1) what should be the value of the stamp paper. 2) can I make the affidavit and get it notarised in maharashtra and use it in another state I.e Karnataka
