Change of name in birth certificate and dob in hbse & kuk

I am 25 year old, but in my school certificates from HBSE and K.U.K degree wrong date of birth is mentioned, and also in My date of Birth certificate wrong Name is mentioned. Please let me know procedure to change d.o.b in school and university certificates and also to correct name in dob certificate.
