Birth certificate changes

Respected all,

I am from SONIPAT (HARYANA) I want to change NAME and DOB in birth certificate. Birth year is 2004. I have contact to the concern dept but there are not ready to change and also not providing the way for making these changes. I have ADHAAR CARD/ RATION CARD/SCHOOL document with actual one. I dont know wht to do? Plz guide me regarding this matter.
