Am trying to apply for fresh passport for my mother. She is having problem with her name, her name is Suguna and during marriage it is changed to Nanjamma. After her marriage all her documents including my sister's passport is with name Nanjamma. For some reason her name is Suguna in my marks cards and I had to use the same name in my passport as well. Now I would like to include her alias name in her passport, please let me know the documents required to be submitted for this and let me know what is the procedure to include alias name in passport.
Note: Since she hasn't completed her school we are submitting affidavit for her DOB in passport application.
