Can someone suggest a draft for making an Affidavit effecting a change in nominee for claiming terminal benefits under Family benefit Scheme operated by a Professional body for its professional members e.g.Indian Medical Association or Indian Society of Anaesthetists

Affidavit should be got attested by a Notary. so consult a local notary and he will prepare the same.

There is no fixed format for it or if there is a printed format by the insurance company you may use it or else you may better take the help of a local notary public who will be able to draft the same and will be able to guide you further on this issue.
