Affidavit for surname

Sir/madam,
Please Advise me.
As my name is priyanka at 10th certificate but it is priyanka singhal in voter and pan card and in bank a/c.I am unmarried.
I want to apply for D/L and Passport.For this DOB certificate and other ID required,so will it cause a problem.Can any affidavit for two person are the same will be acceptable.
I seek Your kind and valuable advise.

No birth certificate at the time of birth.

You can get a birth certificate from the concerned office.

If your birth is not registered, you can get it registered by following the prescribed procedure.

Though such name difference may not create any problem, however, you can get the name corrected in Voter card, PAN Card and the Bank account. It has no a tough procedure.

your name in voter card etc should be the same as in your school leaving certificate .why is your surname not reflected in school leaving certificate

What is your exact name?
If it is Priyanka Singhal why it is not reflected in the 10th certificate.
When you have passed out of that school?
Right now there is no problem; you can apply for driving licence giving Pan Card wherein the correct name Priyanka Singhal with date of birth is available.
But it may create problems especially while applying for Pass Port; during certificate verification for Government employment etc.
The best course is, to apply name correction in your 10th class certificate through the school to the State Board. You can enquire with the concerned District Educational Officer for the procedure.

If born after 26/1/1989, you cannot apply for the passport without the birth certificate issued by the Municipal Authority or the Office of the Registrar of Births & Deaths.

In case of an application for passport, you have to enclose an affidavit in the format of Annexure E and attach it as an enclosure to the passport application.
Note:
In case of change of name, applicant should insert advertisements in two reputed
newspapers (one local newspaper of the area in which he/she is residing and 2nd in
newspaper of the area of permanent address) and submit original newspapers at the
time of applying for passport in his/her new name.
