there are no set performas/ formats for filing cases, cases are drafted as per their merits.

its not a fill in the blanks job

There is no hard and fast prescribed requirements for the second motion petition so leave it to your lawyer. In many places, even there is no need to move second motion as only the statements of the parties are recorded on that day in the proceeding file and further proceeding takes place accordingly,
