Respected Sir,
Will you of any assistance to me in providing me a draft for preparing an affidavit for change in personal name for the purpose of effecting the change in my passport in Mumbai
I have with me the Mahashtra Goverment Gazette followed up with two advertisement in leading newspapers in Mumbai

The format of given by Mr.Sheik is fine,
But the same is for proof of residence and date of birth.
In your case, since it is for change of name, the contents shall be as follows:
After the first paragraph in Mr. Sheikh's draft you shall add
"I state and declare that I was formerly known as Mr. ABC and I have changed my me from ABC to XYZ vide the official gazette of Government of maharasthtra dated __, and, i will be known as Mr.XYZ from ___(insert the date of gazette)
I hereby make this affidavit to produce before the Passport Authority to effect the change in my name in my Passport and in the records of the Passport office
