dear Amit,

there are different format for different states and central govt.

please visit rtiindia.org for all guidance regarding rti act 2005.

practically one does not require and particular format to seek information but few departments have adopted certain formats advised under the rules.

in case u dont find any specific format then write a simple application, mentioning all the details and information being sought by you.

Prepare the application in plain paper setting out the information sought for, address it to  the public infromation officer for the respective department or office, affix the required fee,Rs. 10/ by  DD   or hand over the application at the designated Post office by remitting Rs10/- at the counter  in case the information is sought for from Central Government offices. In case the information relates to State Government Deprtments, ascertain the fee fixed by the respective Government( by way of demand draft or by Postal order) and address it to the Public Information officer . of the respective office. Public information officers have been designated by the Government  both by Central and State Governments in all departments for the pupose of furnishing information under the Right to Information Act 2005.

A very economical versions of RTI Act including illustrations are published and are awailable in the market. These contain the procedure and also a standard formate of applications and appeals, though they are for general understanding and the modifications if necessary (in formates)are also acceptable. Since you are now entering in to this, you will need it tthough out for answers to you questions which may crop up in the process. This is my personal experience as an activitst in this field.

Dear Amit

I would suggest to visit the web site of the public authority from whom u want to seek information. Normally format of application is there in RTI page. However please keep in miond that due to format of application a PIO cannot reject the application as thye Act does not prescribe any format.
