My passport contains old wife name. (separated but not thru court). I won't be able to replace her name with my 2nd wife (legally married now and have marriage certificate).
I am going on work permit outside of India where I want to take my 2nd wife along with me.
What should I do if I want to take her with me outside of India without any hurdles?

Presumably you are professing Hinduism where you cannot marry other woman during subsistence of marriage which is still not dissolved by a decree (of court).
Accordingly your second marriage is illegal and invalid, which cannot be permitted by PP Authorties.
