National lok adalat for corporate entity

My senior is flying down to Chennai for National Lok Adalat of a corporate entity (Fresh Matters). Kindly let me know the procedure for the filing of fresh matters (Of a Corporate entity I.e. a Company and not a Financial Institution) Also the requisite/list of documents needed and if at all the presence of Authorized Representative of the Company is mandatory of not.
