for the the above petition first of all you need to connect with an advocate who will guide you, and coming to mutual divorce both with party consent is needed and where as in contested divorce it's not required . this is just a brief

Both the parties have to present the petition before family signed jointly. 

You may have to attach the proof of marriage,  proof of residence,  any other relevant documents namely,  address proofs of both,  identity proofs of both. 

It is pertinent that you both should have separately for at least one year prior to of filing the petition.
