How should i get my birth cetificate

A Birth Certificate can be obtained by applying through the prescribed proforma with fee as fixed by local body authorities. The certificates are issued from the registration centres/offices of the local bodies in the zone under which the parents of the child were living at the time of birth.

Authority

·         Municipal Corporation

·         Nagar Palika

·         Nagar Palika Parishad

Documents/Papers required

·         Application form with fee.

·         Proof of Birth of the person for whom the certificate is required.

·         Affidavit, if birth is being registered after one year of its occurrence.

i am not registered till yet then how should i register myself and were???

I have a mentally retarded son aged 37 yrs He was borned in Govt Hospital at Kanpur on 08.06.75. Incidentally I have no records with me.How to get his birth certificate .He is severally mentally retarded cannot speak understand or write.
