I am Selvakumar. I tried to apply for the Israel-VISA for my 3 months old daughter
at Mumbai Visa Application Center through our representative. They asked me an affidavit. I do not know, even a lawyer whom I asked also does not know the format. It would be helpful if someone can tell me what are the details must be provided in the affidavit and also a rough format. My email id is acselva@gmail.com.

Im planning to move to France and the visa app. requires a statement from my country of residence to ensure i don't have a criminal record. I currently have a residency permit in the UK, so I need to get it from them, but it all seems impossibly difficult.
