sir i am 18 years old from hyderabd .in my ssc and hsc memo my name is MOHD ALTAF HUSSAIN. but in my birth certificate is is ALTAF HUSSAIN . i want to add MOHD in my name ,please assist me with the formalities i for making recorrection................??????????????

make an affidavit regarding correction in name.....
Publish same in Official Gazette....Regional Newspaper.....
and Go to ur places Municipal Office and submit details there also.....
