How to change my name in qualification certification?

Hi sir,
My name is chikkani Sanjeev kumar.I need to change my Surname in SSC(2008) ,inter & degree .in certificate my name as S SANJEEV.I need to change my name as Chikkani Sanjeev kumar
Now i can change my name? bcoz completed the 10 year.How to change my name? Plz tell me the solution sir...

In addition to the expert, let me add more:-
Better correct it now for your future convenience. Procedure for change of names varies from state to state, but in your case it is in academic certificates, and though it may take time, it is better to start now.
In some States they have made it mandatory to publish in news paper, which is submitted to the concerned authority/Revenue/Collector etc; to published it in the Gazette of India and only thereafter the Institutions are allowed to change the name in the certificate.
Go to Notary and make an affidavit with your reasons why you want changes in your certificate.
1st Step. Make an Affidavit before Noatry, stating specifically the reasons for your desire to correct your certificate
2nd Step.Make two three photo copies of all your vital douments and keep it in a safe folder.
3rd step.File an application address to the Head of of your Board.
4th step.Obtain a forwarding letter from your present Head of Instituion.
5th step.Submit your parents address proof etc, for future correspondence.
6th step.Submit it to your concerned Board enclosing all your photo copies documents/certificates/
This is usually the procedure in our State, publication and gazette is not necessary, and almost majority of States in India do the same, in case of academic certificates etc.
