Dear Pushpakrishna,
Don't take law like a over the counter medicine. Only the Section Number and the Format would not bring the desired result.

Consult a good lawyer for filing appeal. There is no specific format is there. The lawyer would be in a better position to do the job

You can make a search for the format and the provisions of law from google web site itself, put your own efforts before posting the query here.
However the formats alone will not help you file the appeal, the other procedures are also to be known to you so it is advisable to take the help of a lawyer in the local for assisting you on nominal payment.
