Pan card to replace

Dear Sir/Madam,
I have a pan card which was done in 1994. Now want to change to latest plastic card, but dept not accepting the application due to various documents required, like name of father and surname not matching. Ration card name is not valid etc.etc.
Do we have any suggestion to get it done on the basis of the old pan number?

those documents are must for change the card. you approach the nearer private authirsed application provider

Rectify the change in name by publishing in the official gazette of your state government.
Thereafter make an affidavit to that effect and submit to the PAN card authority.

You shall have to fulfil the requirement of the Income Tax department.

The Income Tax Department is now stricly verifying the documents attached with PAN Applications and one has to meet the requirement of LAW. There are number of docments which can be submitted as ID and POA and diffrerent documents can be submitted for ID and POA for example if ou have a document which shows your correct full name and that of your father but has a different address, and also you have another document which shows your name and correct address, this would suffice the requirement. If you dob is correct in original PAN you are not required any evidence of DOB now.

To do so you need proof documents matching with your older one submitted so far they relate to your name,your fathers'name,and your DOB,only address change may be acceptable to them on proof and affidavit.
