Applying for passport and there is an issue of change in surname

Sir,
I have applied a passport and there is an issue of change in surname . My former name is Rama Kumari which is mentioned in my NON ECR supporting documents. Now my present name in new documents such as adhaar uid, voter id,and pan card is Rama Rani. Passport Officer's says to submit Annexure E' on Non Judicial Paper of minimum value.
They also said that to insert advertisement in two reputed newspaper and i have to submit original newspapers at the time of next appointment. {I want to know about the format of writing that is to be print in the newspapers.}

You can engage the services of a local advocate for publishing the item in the newspaper as public notice and proceed with the requirement thereafter.
