UTI bond lost

my UTI bond lost. Bond amount got maturity 2 years back. since I don't have original bond I am unable to claim. kindly help me. tell us the procedure.

Submit an affidavit with bond reference number to UTI office along with an application for duplicate issue of bond papers.
