Trademark documents and type query

I am registering trademark for Name of my company.

Name is a unique coined word derived from 2 separate english words.

I have 2 questions:

1) Should I mention trademark type as  NAME or WORD?

2) What are documents required for TM-51 application?

So please provide the answers, if you know.

Thank you.

We know but things may not be clear for you as the  Fee: Rs.2500/- for each class is prescribed as  a single application for registration of a trade mark for different classes of goods or services(other than a collective mark or a certification trade mark).

 Under Section 18(2), rule 25(9), 103.

This is always better to take Trade Mark Attorney's Advice before you proceed.
