Surname change after marriage for women

As usual My wife's surname has changed after marriage, and we have the marriage certificate, in that clearly mention the name after marriage and before marriage, but every where we are facing the problem, especially RTA, bank account opening, while taking loans etc. Marriage Certificate is not accepting in the mentioned above depts. Please help in this regard.

Remedy:-

1. Give Paper Advertisement under Name Change column. Keep few full page copies of paper insertions. .
2. Followed by Gazette Notification and buy extra copies of the Gazette for all future purposes. Annexure contains the Form for name change for Gazette purposes.
