Query regarding PPSC exam

In the PPSC form there is written " paste recent colour Photograph of size 3.5cm by 4.5cm photograph with name plate".

Does it means that

(1) the photo graph  with holding name plate in hands. Or

(2) photo graph  with name printed below it by computer.
