Problem in obtaining death certificate

Print out the GPay transaction showing the date, time, transaction reference number/UPI Ref ID, and the recipient's name or phone number. Submit a notarized self-declaration affidavit signed by the family (the primary informant).

Meet the Sanitary Inspector (SI) or Registrar of Births and Deaths: Explain that the private driver is extorting money ($₹15,000$) for a paper receipt and that the law requires registration based on the Doctor's Form 4A and VAO Burial Certificate.

Address a written application to the Health Officer / Municipal Commissioner / Zonal Officer explaining the extortion attempt, attaching the GPay proof, Form 4A, and the VAO burial certificate.

Do not pay the extra $₹15,000$. This is illegal extortion.  

Send a clear WhatsApp/SMS message to the ambulance contact:"Payment of Rs. [Amount] was made via GPay on [Date] for transporting the remains. Please send the valid GST/Tax receipt for the amount already paid, failing which an extortion complaint will be lodged with the local police and transport department."

If they refuse, approach the local police station or submit an online complaint on the National Cyber Crime Reporting Portal (1930) or local police portal for extortion using UPI banking details. A simple police intimation copy citing extortion by the driver can be submitted to the Corporation office as proof.

Under Section 13 of the Registration of Births and Deaths Act, if there is a administrative delay or dispute over documentation, the Executive Magistrate / RDO can pass an order directing the Registrar to register the death based on the doctor's certificate and burial proof.
