Passport issue

Hello sir ,
Me and my partner decided to get married so before marriage I add her name in my passport with notrised document of marriage .. but due to some problem we are no longer together . And we never got married .. now my passport shows I am Married and her name on it .. how to change this and remove this from my passport .. please help
