Passport change in name requirements

Sir,I want to change my name in my passport which is issued in India.What are the requirements to be fulfilled ? Can I get my name changed in this passport abroad also where I am residing presently ? Additionally can I get an affidavit prepared and signed by notary public from any place in India whether I am residing at place or not ?
