Need to surrender my EPFO and ESIc licence

I am from guntur Andra Pradesh.

To surrender your EPF and ESI license in Andhra Pradesh, you need to submit a formal application in the designated form ("Form SE-1") to the Regional Provident Fund Commissioner (RPFC) through both email and registered post, clearly stating your intention to surrender the exemption granted to your establishment from a prospective date; ensure to include all necessary supporting documents like Board of Trustees resolution and relevant government notification regarding the exemption granted to your establishment.
