Approach the election branch of the area, it appears that you did not go through the draft voter list published by the election branch. The procedure is the electon branch publishes the draft voter list for the public to invite objectons, since the election is very near in Gujrat, you have to take initiative after the election for correction of the address and inclusion of the name. Every year the process of inclusion of the names are done and in that case make yourself informed of the process.

i dont think every year they will release the draft

you are free top inspect the eixsting ovter list any time . If they do not show easily use RTI.
Youy can seek RTI on the fate of your form 6.
