medical leave and earn leave

i again posting my query . I am a government servent. My doctor gives me 30 days medical leave. But i want to avail more as i have some other work at home. So i apply for 30 days medical leave and in conjunction also applied 10 days earn leave. But employer say that i have to get a fitness certificate and than i can apply for earn leave. Which is correct can i apply for both leave or i have to get an fitness certificate and than i can apply for earn leave?
