If any company having business of consultancy and BPO and banks are client then is that necessary to pass resolution for opening of bank account to claim fees from such bank.

Yes it is imperative to pass board resolution for opening an account, infact bank wont proceed until a copy of board resolution is handed over to them.

Yes.
Each time a Bank Account is opened, a resolution is required.
To add, each time a Bank Account is closed, a resolution is required too.
