Hi Rajnish

Please find enclosed Form 1 under ESI Act.  It is self explanatory. For obtaining ESI code, Form 1 should be submitted to the Regional Director, ESIC of your region duly filled in. 

Similar is the formality for PF.  I shall send you the soft copy of Application form shortly.

no other formality will be done for applying ESIC code.

Hi Rajnish

Along with Form 1 you have to submit the following documents for applying ESI code:

1. List of directors duly attested.
2. List of employees- Name, Fathers name, DOB, DOA, SEX duly signed by the Authorized signatory. 
3. Factory License copy
4. Copy of PAN Number
5. Land allotment order in case of Industrial Estate

After filling the form, take it to the Local office, ESIC and get their input before submitting the same to the Regional Director.
