Index 2 error

I have recently done my Sale dead. I am first owner and my wife is second owner. I haveing sale deed copy in that index 2 having me and my wife both name as owner jointly. recently i applied for second copy of Index 2 in this copy my name is not showing only my wife is showing as owner how I Do rectification I need to do rectification dead for add my name as owner Please help and advice.
