Above is the reproduced item no. 4 of afidavit for name change, I want ot change my name from Sanjay Kumar to Sanjay Sharma, I want to know what shall i fill in the above item no. 4 of the afidavit, I am not able to understand whether it should be my old name (Sanjay Kumar) or new name (Sanjay Sharma) pls guide me in this.
