In my birth certificate my name is mentioned as SenthilKumar and in all other documents (school certificates, ration card, driving license and etc..) it is mentioned as Thiagarajan. Now i cant able to get the passport due to name conflict in my birth certiicate. can you please guide me to do further.
Note: For Gazzette notification i need a document with my old name but i doesn't have any thing in my old name except Birth certificate

Dear Thaigarajan,
I donot understand how your birth name and documents name is different. However you decide which name you prepre. If you want to continue the same as mentioned in the documents i.e.Thaigarajan,If you want to continue the same, with affidivit you can change the name in the birth certificate with the help of advocate. After getting adffidivit, you have pass port easily.
