Help: filing suit of permanent injuction

FILING A SUIT FOR PERMANENT INJUNCTION AGAINST WIFE.

Kindly tell me or gimme a link to:-

How I can file a suit for permanent injunction against my wife and relatives from entering my premises. Do we have to state the grounds for this ?

It may be against her relative but not against her.

Don't invite problems and try some other solutions.

But I have been thrown out of my own house on account of DV.

Now residing separately where she threatens to disturb and create rucus.

SHould I go ahead.
