buy a bare act of the consumer protection act,in the end u ll find the format....not much of the formalities are required there.....but do attach the original order alongwith the appeal.

get a book of consumer protection act, u will get ur reply, u will see the format therein but remember to attach the original copy of the order against which u r appealing alongwith the memo of appeal.
