Documents for gwa case for petitioner in person

Dear All - I am going to file a GWA case in Calcutta and will be petitioner-in-person. Can you please let me know what documents are required to submit the case. As I will fly down from Mumbai so iterations will be very costly and difficult. As of now below are the documents I can think off. Can you please let me know if anything else is needed. I think there is no affidvadit required. Kindly can anyone confirm.
Also, can anyone of you share a power-of-attorney template. I would engage a lawyer for procedural and operations issues as Mumbai is very far. However I would be in-person in front of Hon'ble court so would prefer a power-of-attorney instead of vakalatnama.
 
1. MARRIAGE Certificate 
2. CHILD BIRTH CERTIFICATES 
3.      Address proof of petitioner(s)
4.       ID (PAN Card) of Petitioner(s).

Submit address proof of child/ respondent mother also.Your petition should highlight the points which will take care of the interest of child and how you are a better parent than your wife. Attend weekly meetings organised by MRA group.
