Deed of declaration & stamp duty

I am purchasing 25 year old flat.
That was a old wada and its reconstructed in 1996.
Documents present for that flat are :
Deed of Declaration
Index II for Deed of Declaration

I am applying for bank loan, bank wants registration receipt for Deed of Declaration.
But this receipt is not available with owner.
I tried to get that receipt from local sub registrar haveli but they said its can not be generated since its too old (1996).

My question is how to proceed in this situation ?
Is Stamp duty need to be paid on Deed of Declaration (on Index II Document type is : घोषणापत्र Ghoshanapatra) ?
