Change of name and sirname in passport help please

I want to renew my passport with change of my name and surname.

Please tell me what documents do i require for the same:

My Name as on my old passport is HARESH KANTIBHAI PATEL

My Name i want on my new passport is HIREN KANTIBHAI VIRANI

It would be a great help if someone can guide me on this..

you need to apply and get your name change done via Gazzatte copy issued by the state government. once that is done, you may apply for the passport with your changed name, subject to you annex the gazzatte copy alongwith your passport application.

I have already done that. I have a documents of Gazzatte copy issued by the state government.

And with the help of this gazzatte i have changed my name in all my documents like PAN CARD, VOTING CARD, RATION CARD etc..

But officers at RPO told me that we can not renew your passport with changes of name and surname both. you can only renew your passport with change of either name or surname.

is it true that i cannot renew my passport with changes of name and surname both?

and if not please help me with guide of what should i do next?
