Address proof for rc address update

I want to update my new address in RC book, and they rejected Rental Agreement and Airtel Landline Bill as valid documents. I was asked to get a Self swearing Affidavit by an applicant before an executive Magistrate or a First Class Judicial Magistrate or a Notary Public as evidence of age and address. So, I got in touch with a lawyer and he his asking Affidavit type, I am clueless on this. Can anyone let me know Affidavit type required for the same
