About what should i write in memo?

Dear All,

I field RCR party in person, unknowingly I wrote my address on the Acknowledgementi, eventhough Court stamp was also marked on the Ackonwledgement Card(by default 2 sender addresses were on the Ack. Card)

Later i received the Ack Card to  my address, immly i went to submit the same in Court Section Office, the superintendent rejected of taking it & said "submit a Memo on the next  adjournment along with the Ack Card"  

Now What  should i write in memo:

1) About postman delivering it to me eventhough there was court address marked on the Ack. Card

2) My small mistake about unknowingly writing my address on the me Ack Card.

3) or any other relevant info

ADVICES.......ASAP .....
