As long as the marriage exists,she is the wife of X.hence she cannot avoid mentioning her marital status as married.It amounts to suppression of material facts.

Unless there is no decree of court for judicial separation she cannot mention as separated as well. Any fact suppressed to obtain passport or visa may attract severe punishments as per criminal law.
